Tuba City Unified School District Immunization Guidelines for Grades K-5
Dear Parents of Tuba City School District Students:
We are facing momentous events due to COVID-19 and hoping all is well with you and your family. Though the school has new methods of instructions due to the corona virus we still will require your child’s updated immunization records.
The following is a general guideline for K – 5th grade (by age)*:
- Age (4-6 yrs old) should have:
- Hep B/[HBV] (3 doses);
- Poliomylitis/Polio (4 doses) or (3 doses acceptable if dose #3 was received on or after 4 yrs old);
- Measles, Mumps and Rubella [MMR] (2 doses);
- Varicella [chickenpox] (1 dose);
- Diptheria, Tetanaus and Pertussis [DTap/DTP/DT] (5 doses) or (4 doses acceptable if last dose was given on or after 4 years of age).
- Age (7-10 yrs old) should have:
- All the above and Tdap or TD] (1 dose)
*Note: Vaccines must follow minimum intervals and ages to be valid. A 4 day grace period applies in most students:
Please arrange with your family pediatrician to get these immunization shots. Local immunization shots are available at the Tuba City Pediatric Clinic 928-283-2679 or Toll free 1-888-264-9905.
You must make an appointment a week in advance; shots may be administered via drive thru. All present must wear mask unless 2 years or under. For those who choose NOT to get vaccinations for medical purposes or personal belief, you MUST FILL OUT AND SIGN EXEMPT FORM. Exempt forms will be available at the school’s front office; please ask the school’s secretary.

Please submit updated copy of your child’s Immunization Record as soon as possible to your school’s front office..
