District Vaccine Clinic – Monday, September 28


Dear TCUSD Staff,
Tuba City Unified School District, in partnership with ASBAIT and Diversified Solutions Occupational Health, will host an onsite vaccine clinic for employees and eligible dependents.

The clinic will offer flu vaccines, as well as COVID-19, MMR, Tdap, pneumonia, and shingles vaccines for individuals who meet the applicable recommendations and eligibility requirements. Participation is completely voluntary.
Please review the attached clinic information carefully before attending. Employees planning to participate should:

Bring their current insurance card.

Bring a completed consent form or complete one at the clinic.

Coordinate with their supervisor to ensure appropriate coverage and avoid disruption to student instruction and district operations.

Plan to remain near the vaccination area for approximately 15 minutes after receiving a vaccine, as directed by the provider.
Vaccinations are available at no cost to employees, and eligible dependents covered under most ASBAIT medical plans. The provider also accepts several other insurance plans, including certain Blue Cross Blue Shield, UnitedHealthcare, Medicare, and Tricare plans. Coverage varies by plan, and submission of an insurance claim does not guarantee coverage. Individuals without insurance may pay the applicable cash price; the listed cash price for the flu vaccine is $60.
Flu vaccines are available to eligible dependents ages five and older. Please consult the attached information or your healthcare provider regarding eligibility for the other vaccines offered.
